Probe panel in a fix over cause of VSP fire

In the absence of records on pressure regulating system (PRS)operation and changes made in the operating regime between the second trial and the time of fire at VSP on June 13, it is difficult to pinpoint what actions were actually taken by the persons present at the control room when the incident took place, the inquiry committee has felt.
The committee, in its report released on Friday, observed that the flow rate increased beyond the set value of 450 Nm3/min during the fourth blow and it was maintained during fifth blow as well.
